Pick up shifts as needed - Days, Evenings, Weekends (no midnights)

Requirements

Current State license as a Licensed Practical Nurse (LPN).
Graduate of an accredited School of Nursing.
1+ years of experience in a home health care setting or facility.
Excellent communication skills and time management.
Current CPR certification or ability to obtain.

Responsibilities

Provide nursing care as defined in a home health care setting.
Recognizes and reports condition changes and follows plan of care.
Take physicians orders or medical personnel/record.
Communicate information in a timely manner and consistent with home health policy and urgency.
