Adult Day Health Coordinator / Supervisor (Education)
Orients new staff to the Adult Day Health Regulations and applicable local regulations.
Provides departmental orientation and ongoing training for Adult Day Health staff, students, volunteers and contract providers assigned to the Adult Day Health Center.
Assists with policy and procedures solutions as they relate to performance and productivity of the agency and staff.
Serves as liaison between administration and direct reports.
Ensures nursing assessments are conducted per laws, regulations and agency policy and procedures.
Adult Day Health Center (ADHC) Health Coordinator
Serves as the ADHC Health Care Coordinator.
Maintains policies and procedures in accordance with ADHC regulations.
Assists in monthly site visits (as needed) from ADHC auditors.
Directs participant's care based on nursing assessment. Treats and/or refers to appropriate discipline for further evaluation.
Engages in direct communication with the Clinic Supervisor when triaging participants who require a clinic visit to include timely documentation to PCPs and appropriate IDT team members per standard operating procedures.
Uses research, nursing process and collaboration to solve identified problems
Engages in and prepares assigned Certified Nursing Assistants to provide input in participant care planning.
Ensures accurate and timely follow through of orders related to care and services in the Adult Day Health Center.
Provides timely and effective communication and follow up with participants, families and contractors.
Assures Adult Day Health representation at Morning meetings and IDT for care planning purposes.
Communicates with families and participants, connecting them with appropriate resources internally and externally.
Operations and Quality Management
Manages all Adult Day Health daily operations including but not limited to participant needs including socialization, engagement, life enrichment, meals, personal care services and ensures Certified Nurse Assistants provide excellent service.
Serves as point person for Adult Day Health vendors.
Assists in the development and monitoring of departmental budget.
Works with disciplines to develop best practices for daily procedures by creating templates, forms, checklists, and step-by-step standards.
Assures compliance with all federal, state and local laws and regulations.
Actively participates in organization's compliance and quality committees.
Collaborates with Quality Coordinator on performance improvement projects.
Completes written reports required for job duties.
Maintains equipment according to manufacturer's recommendations.
Coordinates ordering of Adult Day Health supplies and equipment needed for safe participant care in a cost-effective manner and related authorizations if needed.
Other Duties include but not limited to:
On-call rotation as assigned by Director of Clinical Services.
Attends staff meetings.
Assist with caregiver and staff education.
Actively participates in ADHC operations to develop, implement, and revise policies and procedures and ongoing operational decisions related to the agency.
Other duties as assigned.
Environmental Factors
Tasks with potential for exposure to blood borne pathogens include, but are not limited to the following:
Assisting with mouth care
Assisting with care of non-intact skin
Assisting incontinent participants with personal care
Administering tube feedings PRN
Shaving participants
Assisting with procedures in the clinic
Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
