Kidney Care Manager (Finance)

The Nurse Practitioner/Registered Nurse - Kidney Care Manager is a member of the Care Management team in the Value-Based Care division of Satellite Healthcare. The kidney care manager will be responsible for providing care and case management for patients, and delivering clinical care to chronic kidney disease and end-stage kidney disease patients. This position will be working closely with complex renal patients in their home, dialysis center, by phone, virtually, and electronically as needed. The goal of the care management team is to improve the quality of patient care, improve patient outcomes, and lower costs. This is a market-based position in the Greater Modesto area of California's Central Valley. The kidney care manager will work with patients and the care team through in-person, virtual, telephonic and electronic means of communications, care coordination and management. As a member of the care management team, the kidney care manager will work with other field-based clinical staff and physicians, non-clinical staff, and interact with providers, hospitals and ancillary services in the Bay Area of Northern California.
